個別訂購需負擔個別抗體進口手續費，請確實填寫項目，並於截止時間時間內送出，謝謝。

中心配合國科會的指導，代訂以原價收費。
分子試劑訂購來源：
1. Antibody (來源: The Developmental Studies Hybridoma Bank )
2. Clone / Vector (來源: The Drosophila Genomics Resource Center )
3. 其他項目或來源請依表單內容適度修改並來信註明

表單填寫完畢後將自動生成使用同意證明單，請於確認金額及品項正確後，填寫計畫經費來源、扣款計畫編號及扣款計畫主持人欄位，並勾選經費來源類別，由實驗室主持人簽名彩色掃描回傳。
